To enroll in a health insurance plan at NY State of Health, you’ll need:

  • Social Security card or Permanent Resident Card
  • Proof of income, such as a W2, 1099 form or paystub, and your employers address and phone number
  • List of your current doctors and medicines
  • Create a Marketplace account
